Preparation For Your Appointment

Thank you for scheduling an appointment with our doctors at the Pennsylvania Centre for Dermatology.  We strive to provide patients with top-notch dermatology care in a compassionate and professional environment.  In order to optimize your care and provide the best time-management during your visit, we ask that you abide by the following guidelines:

Please arrive 15 minutes before your scheduled appointment time.

In order to save time, please complete your registration materials prior to your visit (see next section for links to these forms).

Please make sure that you bring the following items with you to your appointment:

  • Insurance Card
  • Referral form (if required by your insurance company)
  • Cash, check or Visa/Mastercard for your co-payment. (There will be a $50 fee for returned or cancelled checks)

Please remember your co-pay for this visit.  We do not bill for co-pays.  This is a requirement of your insurance company.  If you are unable to keep your appointment, please notify us at least 24 hours before your scheduled appointment.

There will be a cancellation fee of FIFTY DOLLARS ($50) for appointments cancelled with less than 24-hour advance notice.

In order to facilitate the treatment of your skin condition, please request that all pertinent medical records be sent by your referring physician prior to your appointment.  Please complete the form below and fax to your physician.  This form authorizes the release of this information to the Pennsylvania Centre for Dermatology.

Thank you for participating in our patient portal. Our goal is to provide our patients with the best communication options possible.  Toward this goal, effective March 26th, we have upgraded the platform.  Below are the steps to request access to the new portal.

  1. If you have an upcoming appointment, we can provide you with your new log in while you are visiting the office
